Privacy Policy Notice

Patient Advocate Foundation is committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you can be identified when using this website; you can be assured that it will only be used in accordance with this privacy statement.

This Privacy Policy provides an explanation as to what happens to any personal data that you provide to us, or that we collect from you.

We may change this policy from time to time by updating this page. You should check this page from time to time to ensure that you are happy with any changes. This policy is effective from May 2012.

Information We Collect

  1. Details of your visits to our website and the resources that you access, including, but not limited to, traffic data, location data, weblogs and other communication data.
  2. Information that you choose to provide by filling in forms on our website, such as when you register a request for information or choose to subscribe to a PAF distribution list.
  3. Information provided to us when you choose to communicate with us for any reason.

Use of Cookies

On occasion, we may gather information about your computer for our services and to provide statistical information regarding the use of our website to our advertisers. Such information will not identify you personally it is statistical data about our visitors and their use of our site. This statistical data does not identify any personal details whatsoever.

Once a website visitor initiates ongoing patient services with Patient Advocate Foundation, your information is treated in accordance to our organizational patient privacy policy that addresses information beyond web-based interaction. Please be advised that we do not reveal information about identifiable individuals to our advertisers, donors, grant providing organizations or partners but we may, on occasion, provide them with aggregate statistical information about our visitors.

Storing & Securing Your Personal Data

Patient Advocate Foundation goes to great lengths to protect the confidentiality and integrity of any personal information you choose to share with us on the web.  To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we strive to maintain physical, electronic, and administrative safeguards.  When you submit personal information via our website via an online form or web chat, your information is protected both online and offline.  When personal information supplied voluntarily by you is collected, it is encrypted and transmitted to us in a secure way using Secure Sockets Layer (SSL) encryption.  SSL is the standard security technology for establishing an encrypted link between a web server and a browser. This link ensures that all data passed between the web server and browsers remain private. SSL is an industry standard and is used by most websites in the protection of online transactions with customers.   You can verify this by looking for a closed lock icon at the bottom of your web browser, or looking for "https" at the beginning of the address of the web page. When you click on the lock icon you can view our SSL certificate which allows a secure connection to be created. While we use SSL encryption to protect personal information provided by you and transmitted online, we also protect your information offline.  Access to personally identifiable information is limited to only certain staff members who have a job-related reason to have access.  Personal data that is provided to us by you is stored on secured servers that are password protected and kept in a physically secure environment behind enterprise firewall technology.  Our enterprise backup systems use SSL encryption for backup data while in transit and 128-bit Blowfish encryption for data at rest.

The transmission of information via the internet is not completely secure and therefore we cannot guarantee the security of data sent to us electronically and transmission of such data is therefore entirely at your own risk. Where we have given you (or where you have chosen) a password so that you can access certain parts of our site, you are responsible for keeping this password confidential.

Disclosing Your Information

Where applicable, we may disclose your personal information to any official member within our nonprofits corporate structure, as it is necessary to provide you with the best service and customer service addressing your requests. We will also disclose your personal information to third parties, only in the following circumstances:
