Open enrollment is the yearly period when people can enroll into a health insurance plan. There will be information that you will need to provide when completing the application. You will be required to provide at least the following information when applying:
Social Security Numbers (or document numbers for legal immigrants)
Birth dates for you and family members
Employer contact information & wage information, if any, for each household member
Information about any additional sources of income, if any
Information about any current health insurance policies, including this information for children or other family members who will be covered
