Request a Change to Your Drug Formulary

If you are prescribed a drug that is not covered under your drug formulary you may formally request that it be added.

To do so first check with your doctor that there is not an alternative drug available that is covered by your formulary. If no alternative exists, you can have your doctor write to your insurance company requesting that the drug be added to the formulary. Your doctor should provide copies of peer reviews medical literature to prove the effectiveness and necessity of the requested medication.

The pharmacy and therapeutics committee that decides what goes on the formulary will review the request and make a decision.
