Your Pre-Enrollment Checklist

You’ve done your research and determined what the best plan for you and your family is. As you sit down to apply or renew your healthcare coverage, you’ll find that most enrollment applications might ask you for some information you may not know off the top of your head. To avoid getting “stuck” while you’re applying, be sure to take a peek at this checklist so you’re prepped and ready to go!

  • Proof of Identification: You’ll need proof of U.S. citizenship and/or immigration status, identity and date of birth. Your U.S. passport or Certificate of U.S. citizenship are examples of approved documents.
  • Proof of Home Address: Proof of your home and mailing address must be provided for you and anyone you are applying for. This can be a utility bill or recent driver’s license.
  • Proof of Income Status: You’ll need to provide current (and next year’s estimate) proof of income status for you and anyone you are applying for. You can use your pay stubs, award letters or benefit statements.
  • Proof of Health Insurance: You will need to provide Policy and Plan ID Numbers for current (if applicable) healthcare insurance plans for household members. An insurance card or health insurance termination letter can be used.
  • Social Security Numbers: Before applying for health insurance, be sure to have your (and anyone you’re applying for) social security numbers on hand.

Some insurance companies may require additional information but this checklist should give you a pretty good head start on some of the documentation you may need
