Gathering Your Information for Enrollment
Open enrollment is the yearly period when people can enroll into a health insurance plan. There will be information that you will need to provide when completing the application. You will be required to provide at least the following information when applying:

- Social Security Numbers (or document numbers for legal immigrants)

- Birth dates for you and family members

- Employer contact information & wage information, if any, for each household member

- Information about any additional sources of income, if any

- Information about any current health insurance policies, including this information for children or other family members who will be covered
